Request copies of your own records
You can do this yourself. Sign up for an eServices account if you don't already have one. Once you are logged in, you can view your unemployment claim and wage details. You can see online:
- Your unemployment claim information as far back as 2016.
- Wages reported by your employers as far back as 2005.
- Unemployment claim information for your former employees, if you are an employer.
For individuals
You can request copies of your own records if you need or want ESD to send you the records instead. Use this form:

For employers
You can request copies of your records of unemployment claims using this form:
- Authorization to Release Records - Employer (PDF, 146KB)

Third-party program records requests
An individual or a third-party can submit an authorized signed records request online for:
- A copy of an individual's wages reported by state of Washington employers.
- An individual's unemployment payment history.

You can also request authorization to receive copies of an individual's records using this form:

Public records requests
When the Employment Security Department (ESD) receives your request for public records, we will let you know within the five-day statutory period. At that time, we also will give you an estimated date for providing records.

Reviewing records in person
If you want to review records in person, you must email recordsdisclosure@esd.wa.gov to schedule an appointment. This will ensure the records you are requesting are available when you arrive at ESD headquarters at 212 Maple Park Avenue SE in Olympia.
