Missed claims

If you can't file missed weeks online or by phone:

1. Complete the unemployment insurance claim form (PDF, 292KB).
2. Fax your completed form to 800-301-1796.

Note: You must submit this within 4 weeks of your claim's effective date, most recent reopen, or last weekly claim.

Training program forms

Use these forms to apply for training programs.

• Use the application for commissioner-approved training (PDF, 61KB) to apply for commissioner-approved training (CAT) to receive regular unemployment benefits while you attend training.
• Use the application for training benefits (PDF, 172KB) to apply for training benefits to receive extended unemployment benefits while you attend training.
• Use the request to modify your training plan (PDF, 82KB) to request a change to your approved plan for training benefits.
• Use the application for self-employment assistance program (PDF, 29KB) to apply for the self-employment assistance program (SEAP) to receive unemployment benefits while you attend training to start a business.
