Missed claims

If you can't file missed weeks online or by phone:

1. Complete the unemployment insurance claim form (PDF, 292KB).
2. Fax your completed form to 800-301-1796.

Note: You must submit this within 4 weeks of your claim's effective date, most recent reopen or last weekly claim.
