Requesting locked services in EAMS

Employees of the Business or Tax Preparers

  1. At the 'Available Services' screen, select your business.
  2. Under the 'Administrative menu', choose 'Request access to locked services.'
  3. From the 'Business Owner/Officer' screen select 'No' if the role doesn’t apply and select 'next.'
  4. At the 'Authorization Role' screen, select 'Employee of the business', '3rd party preparer' or 'Professional tax preparer.'
  5. Then choose 'next.'
  6. To complete your request for access, see 'Paper approval,' select 'print.'
  7. Then sign the Employer Account Management Authorization form.
  8. Next, have the owner/officer of the business complete the form.
  9. Fax the completed form to 855-952-9975. Please allow one week for a response.
  10. If approved, you will receive a confirmation email. If not approved, you will receive an email requesting additional information.

  

Owners or Officers of the Business

  1. At the 'Available Services' screen, select your business
  2. Under the 'Administrative menu', choose 'Request access to locked services.'
  3. From the 'Business Owner/Officer' screen select 'Yes.' If you’re on record in our database as the owner or an officer, you will receive instant approval.
  4. If you’re NOT on record with our database, you will need to complete your request for access, print and sign the Employer Account Management Authorization form.
  5. Then fax the completed form to 855-952-9975. Please allow one week for a response.
  6. If approved, you will receive a confirmation email. If not approved, you will receive an email requesting additional information.
